A July 7 autopsy report obtained by PEOPLE on Wednesday, Aug. 4, stated that Adam had fatal levels of fentanyl, a powerful opioid, in his blood. He also had traces of flualprazolam, a tranquilizer, mitragynine, a psychoactive ingredient found in the kratom plant, ethanol, and cocaine in his system. The report ruled his death as accidental and said he died at his residence in Los Angeles.
